If you are under 18

If you are under the guardianship of the Secretary of the Department of Health and Human Services and you wish to leave your home or your placement before you turn 18 you should speak to your Child Protection Worker. 

If you are over 18

If you are over 18 you can leave care and make the transition to independent living.

 

If you are over 18 you are entitled to a Leaving Care Plan

This plan will include ways you can be supported through after-care services to deal with any issues that may arise when you leave care. This is designed to reduce the stress involved in leaving care and learning to live independently.

 

Your Leaving Care Plan should include:

1.    Family support. Information and advice about other agencies that can help you with family searching and family mediation.

 

2.    Housing. Financial assistance and support to help you find accommodation.

 

3.    Education. Assistance in finding the right education and training for you.

 

4.    Employment. Assistance in finding employment.

 

5.    Counselling. Counselling and other services.

 

6.    Documentation. Help with accessing personal documentation.
