You are entitled to a Leaving Care Plan
Your Leaving Care Plan should include:
1. Information and advice about other agencies that can help you with family searching and family mediation.
2. Financial assistance and support to help you find accommodation.
3. Assistance in finding the right education and training for you.
4. Assistance in finding employment.
5. Counselling and services.
6. Help with accessing personal documentation.

My Stuff – Leaving Care Kit
The My Stuff Kit is a planning tool and resource folder to help you when you are making the transition out of care. Ask your case worker for this kit.
How will the kit be used?
The kit is designed to be used by you with the support of your carer, your social worker, a youth worker, or a trusted member of your family.
As each topic is explored, you will build your own information store about services, agencies and people who can assist you in all areas of your life. You will also gain important knowledge about day-to-day living such as buying food, cooking and budgeting.
